Anxious to save Casey's life, Valero goes into investigation about what's been happening to her since her trip to the Twin Lakes with Quinn.  He learns more about the prophecy of doom:  She is destined to die because somehow, Quinn will impregnate her and since Humans cannot handle the strain of giving birth to Niflheim, she will die in the process.  At once, he goes on to investigate her relationship with Quinn, but hits a dead end when Lex flatly tells him that the two are not lovers, just friends.  At the same time, Casey learns the news of her impending doom and resigns herself to die, a fact that upsets Lex very much.

Determined to save her best friend's life, Lex attempts to talk Casey into returning to Beigor for a second hysterectomy.  When Casey refuses, Lex instead turns to Ariadne, asking her to guide them to Casey's family so Lex can learn the name and location of the nefarious doctor who performed the surgery the first time around.  A curious Valero sneaks off after them and watches from the shadows as they face Queen Darla, her brother Trey, and Casey's pushover mother, Princess Laura.  Lex, Ariadne, and Valero are all amazed at the disinterest of Casey's parents when they learn that her life may be in danger, but finally, Trey gives them the information they need about the healer they're seeking.

Returning the House, Lex is determined to leave for Beigor.  Ariadne warns her that Casey will never go willingly.  Lex agrees, but is unable to find a peaceful solution.  At last, Ariadne orders Lex to call Casey out to the lobby and keep her distracted.  While Lex tries once more, unsuccessfully, to talk Casey into getting the surgery, Ariadne sneaks into the kitchen.  She returns with a cloth soaked in ether and applies it over Casey's mouth and nose.  Casey falls unconscious, allowing Ariadne and Lex to bundle her away.  Having witnessed the entire display, Valero follows them as they journey all the way to Beigor.

With no trouble at all, Lex and Ariadne manage to find the surgeon, named Admetus, who runs a clinic with his assistant Shaheen.  After paying an obscene amount of money in fees and bribes from Lex's own accounts, Admetus and Shaheen take Casey into a back room.  Valero arrives at the clinic and confronts Lex and Ariadne about their actions.  Lex passionately defends the decision to bring Casey.  Meanwhile, in the back room, Shaheen and Admetus rejoice in their improved fortunes.  Casey wakes, however, to find Admetus molesting her.  She goes into a complete panic and tries to escape, recognizing her surroundings.  Admetus tries to stop her and the two of them struggle.  In the fight, Casey gets hold of a knife and inadvertantly stabs Admetus in the heart.

In a rage, Shaheen runs from the operating room, wielding a large knife.  She immediately blames Lex and Ariadne for Admetus' death and attempts to injure them.  Valero comes to their defense and fights back, killing Shaheen when she falls on her own knife.  The three of them rush into the back room where they find Casey and Quinn at the scene of the crime, Admetus' blood still pouring out.  Immediately, without hesitation, Quinn takes the credit for murdering Admetus.  The shock slowly settles down and Casey surprises everyone when she asks Valero to perform the surgery on her.  Valero agrees and the operation is a complete success.

During her recovery, Casey speaks with Lex.  She tells her that she should not have done what she did and that, by rights, Casey should break off their friendship.  Lex replies that she knew it was a possibility, but would rather have Casey alive and mad at her than dead.  Casey says she knows and that's the reason she agreed to go through with the procedure.  Together, Lex, Valero, and Casey come up with an elaborate lie to explain their leave from the House.  At the same time, outside in the waiting room, Ariadne observes Quinn's deeply troubled meditations, but is unable to understand or do anything about them.
